Parent/Guardian Consent Form (Under 18)
Important Information
If your child/young person would like to take part in this event without a parent or guardian being present and they are under the age of 18, a parent or guardian must read and complete this form to give their permission.
As a parent or guardian, it is your responsibility to ensure you child/young person travels safely to and from the event and whilst Sunderland Culture will complete a registration of attendance at each session, we cannot be held responsible for informing parents/guardians about non-attendance.
Risk assessments are carried out for all events and activities and can be provided on request.
As a parent or guardian of the named child/young person, you acknowledge that you have given permission for them to participate in Sunderland Culture activities.
A copy of our Child Protection Policy and Procedures is available upon request.
Every child/young person will be expected to adhere to our positive behaviour code of conduct. Should the named child demonstrate unacceptable behaviour during Sunderland Culture activities, they could be asked to leave. In this instance, you will be contacted on the emergency contact details below.

Photographic and Film Consent
Please read and ensure you are fully aware of the contents of the document before you sign.
As an event participant your child/young person may be photographed and/or recorded on video for evaluation or future promotion purposes. Sunderland Culture may choose to use this material for publicity purposes and these images may appear on social media sites just as Facebook, Twitter and Instagram, in local or national newspapers, or on televised news programmes.
To comply with the UK Data Protection Act 1998, your permission is needed before we are able to use an image or any likeness of your child. No image or likeness will be used without your permission.
If it is given, you may remove your permission at any time by contacting the person named at the bottom of this document.
The form must not be signed by a minor (anyone under 18 years). Permission for a minor must come from a parent or legal guardian.
